Installing Feebdack Form
Installing this script is very quick and easy. Full instructions are included in the download file however, there is only one file to edit.

Open feedback.php and find youremail@yourdomain.com, change it to your own site email address.
This appears three times on Lines 58, 73, 74

Upload all files to your server and test the script using the demo form provided: form.php
http://www.yourdomain.com/form.php

Your form should be fully working.
